Address: 1116 S. Dobson Road, Mesa

Major violations: 9

Inspector’s comments: Cooked rice not held at 130 degrees or above; employees did not wash hands during inspection; raw shrimp stored above ready-to-eat pickled vegetables in reach-in; ready-to-eat flan and tiramisu in front case not properly date-marked; kitchen restroom noted in disrepair, sink has been removed from wall and drain tube noted discharging into toilet bowl tank; chemical spray bottle stored on shelf next to and above clean utensils; bottle of glass cleaner improperly labeled sanitizer; the walls, floors and coving in poor repair throughout the kitchen; numerous wiping cloths on the tables and not stored in sanitizer between uses; heavy buildup of dirt and grease in kitchen under and around equipment; numerous lights out in kitchen and storage room and missing shields; scoops in use stored in ice water between uses, knives stuck between tables between uses; no paper towels in kitchen; wet towels in contact with numerous ready-to-eat foods; no certified manager present at time of inspection; buildup on slicer and utensils stored with other clean utensils.

Inspector’s comments: Large pan of cooked beef in walk-in cooler had blood pooling on foil lid and foil was torn and not sealing food from raw product; there was a galvanized steel bucket in walk-in cooler with cooked tamales inside; several spice containers did not have lids and plastic cup was used to scoop spices; large amount of cooked pork in hot holding unit at 108 degrees and cooked fish in hot holding unit at 105 degrees, not held at 130 degrees or above; tube of burn cream stored over reach-in deli case; employee handled tortillas and placed in bag at prep table with bare hands; live baby roach on reach-in deli case.

Inspector’s comments: A 10-inch-deep plastic container of cooked noodles was cooling in the walk-in cooler and another container of rice was cooling on shelf, not cooled to the proper temperature within the required time and temperature range; an open employee drink was stored on a kitchen shelf above rice, drinks must have a lid and a straw; there was dried organic matter on the surface of the slicer blade from the last time it was used; there was no food service manager on staff; a lot of mouse droppings found on the lower shelving in the front service area and a live German roach found on a glue trap; an employee cell phone was on a shelf by the prep table in the back of the kitchen; the hot water handle is not working on the faucet of the food prep sink.

Inspector’s comments: Employee washed hands in the restroom then proceeded to put on gloves, hands must be washed at the designated hand wash sink; employee water bottle was at the front counter, also walk-in cooler had a bottle of half-filled Gatorade on the top shelf, drinks must have lids and straws; sliced ham not correctly date-marked, had date of July 23 and was reportedly sliced yesterday; gyro meat in glass cooler was not date-marked; there were numerous flies in the kitchen; 2-inch brush with a metal band in oil is not approved for use in food production area; upright freezer had frost buildup, had frozen food and bread inside, this freezer is not approved; green racks in the walk-in cooler had organic mold on the underside of the racks; bowl of fresh salad in the walk-in cooler was uncovered.

Inspector’s comments: The cutting boards and the knives on the food prep serve line were soiled; chemical sanitizer was not at the proper concentration; chili had passed the discard date of Aug. 3 in the walk-in cooler; spray bottles of chemicals not labeled; trash bags used for ice storage; grounds behind the kitchen were wet due to sewage being discharged.
