Address: 6316 N. Scottsdale Road, Scottsdale

Major violations: 6

Inspector’s comments: Cooked sausage patty on cook line holding at 95 degrees, not held at 130 degrees or above; reach-in cooler had cheese, tuna salad, raw chicken and hamburger not holding at proper temperature; ice scoop was laying in ice with handle in contact with ice; servers do not wear gloves; following items lacked date mark in walk-in cooler: mashed potatoes, cooked turkey, cooked ribs, cooked rice, prime rib, cooked pasta; backflow preventer at dishwashing area near garbage disposal broken, missing its cap; reach-in coolers on cook line had seals in disrepair, cooler not holding at proper temperature.

Inspector’s comments: Cook used bare hands to prepare ready-to-eat foods; raw shell eggs stored over ready-to-eat foods in the make table at the cook line; interior of the roll-top for the make table at the cook line had food debris, cooked pork found in the walk-in cooler without date marks; reusable latex gloves and cleaning supplies found in the back hand-washing sink; rice spoons found in standing water; not all employees had food service worker cards; thawed fish found at 72 degrees; lids to food containers reused without washing between uses; filters for grease extraction hood needed to be cleaned.

Major violations: 5

Inspector’s comments: Raw shrimp was being thawed sitting in water at 70 degrees to 86 degrees; chemical sanitizer not at proper concentration; white insert used for keeping potentially hazardous food items cold was severely cracked at the lip of the container; pasta portioned before the cooling process was done; sprayer under pressure attached to water line sitting down below flood line of dump sink in bar must be removed.


Address: 1470 E. Southern Ave., Tempe

Major violations: 5

Inspector’s comments: Window cleaner was found stored with drink cups in the bar; the bar drink dispensers, the slicer, pizza utensils, the can opener, the pizza oven, the unused refrigerators were all dirty; bottles were being stored in the drink ice; broken toilet seat in the men’s restroom; in the walk-in refrigerator, roast beef, cooked meatballs, pepperoni and refried beans were not dated; raw meat was stored with cooked meat in the walk-in refrigerator; bags of dough were on the floor in the walk-in refrigerator and in the walk-in freezer.

Inspector’s comments: Live roaches in the establishment; container had dry food debris buildup inside; wiping cloths stored on kitchen counter between uses; cabinets by the wait station in need of cleaning; food in the walk-in refrigerator and in the freezer not covered; big hole under dishwasher area; cardboard boxes, unused equipment and containers stored in the establishment; many wood shelves in need of paint or replacement; dead roaches under cabinets and on the floor.
