(AP) — Kevin Kolb is inactive for the Arizona Cardinals for the third straight game with a right foot injury, and he's being replaced at quarterback by John Skelton against the San Francisco 49ers.

A turf toe and a bruise on the side of his foot will keep Kolb sidelined again Sunday. He tested the injuries briefly in practice this week but complained of pain when he planted to throw or moved around.

Arizona tight end Todd Heap also is inactive with a hamstring injury.

The 49ers are relatively healthy.

Running back Frank Gore's right knee injury and defensive lineman Ray McDonald's hamstring strain had each listed as questionable this week. Both are in the starting lineup.
